Weight-Loss Jabs, Muscle Loss and Strength Training: How Boxing Can Help

People taking weight-loss jabs such as Mounjaro and Wegovy are now being specifically advised to include activities that help maintain muscle strength. Updated UK Chief Medical Officers’ guidance, published on 10 July 2026, highlights strength and balance exercise for people using GLP-1 medicines for weight management or diabetes.

Weight-loss injections can support significant reductions in body weight, but some of the weight lost may come from lean tissue as well as body fat. Lean mass is not identical to skeletal muscle, and the evidence does not mean that medicines such as Mounjaro or Wegovy simply “eat muscle”. It does mean that progressive resistance training, appropriate nutrition and regular physical activity should not be ignored.

I’m Paul Carroll, founder of Boxing Trainer London and an independent personal trainer and private boxing coach in London. I hold a BSc (Hons) in Sport Science, am a former British University Heavyweight Boxing Champion and former Great Britain international decathlete, and have more than 25 years of experience coaching boxing, strength, conditioning and fitness.

My role is not to prescribe weight-management medication or provide medical treatment. My role is to translate the exercise guidance into practical training: combining boxing, strength exercises, cardiovascular fitness, movement and gradual progression around the individual.

If you are searching for Mounjaro strength training, Wegovy exercise, GLP-1 muscle loss or boxing while taking weight-loss jabs, this guide explains the evidence, the limitations and how a properly structured boxing and resistance programme may help.

For the wider fitness approach, read my guide to boxing for weight loss in London. Complete beginners can also start with Boxing for Beginners London.

Weight Loss Jabs and Strength Training – Quick Answer

  • Should you strength train? Yes. Updated UK guidance says people taking GLP-1 medicines especially need activities that help maintain muscle strength.
  • How often? Adults should strengthen all the major muscle groups on at least two days each week.
  • Can weight-loss jabs reduce muscle? Weight loss can include reductions in lean mass as well as body fat, but lean mass is not the same as pure skeletal muscle.
  • Does boxing help? Boxing can improve cardiovascular fitness, balance, coordination, movement and exercise consistency.
  • Is padwork enough? Not by itself. Boxing should be combined with progressive exercises for the legs, hips, back, chest, shoulders and trunk.
  • Which brands are covered? Mounjaro, Wegovy, Ozempic and Saxenda, alongside tirzepatide, semaglutide and liraglutide.
  • Can complete beginners start? Yes. Training should begin gradually and be adapted around health, fitness, symptoms and clinical advice.
  • Training cost: private 1-to-1 coaching is normally £75–£95 per session depending on location and package.

The goal is not simply to become lighter. It is to maintain useful strength, improve fitness and build an exercise routine that can continue long term.

What Is the New UK Advice on Weight-Loss Jabs and Strength Training?

The updated UK Chief Medical Officers’ guidance says people taking GLP-1 medicines for weight management or diabetes especially need activities that help maintain muscle strength.

The refreshed guidance was published on 10 July 2026. It retains the established adult activity targets but places renewed emphasis on strength, balance, light daily movement and reducing prolonged periods of sitting.

The government announcement states:

“Those on GLP-1 agonist drugs to combat obesity need to maintain muscle strength.”

— Department of Health and Social Care, 10 July 2026

The guidance recommends that adults:

  • Are physically active every day where possible
  • Strengthen all the major muscle groups on at least two days each week
  • Accumulate at least 150 minutes of moderate activity each week
  • Alternatively complete 75 minutes of vigorous activity or an equivalent combination
  • Reduce long periods of sitting
  • Break inactive periods up with movement
  • Include activities that develop or maintain strength, balance and coordination

The guidance also reinforces an important point for beginners: the biggest health gains often come from moving from inactivity to any level of regular activity. You do not have to begin with exhausting circuits or maximum-effort workouts.

Why This Became a National News Story

Reporting on the new advice, The i Paper highlighted the large number of adults now using weight-management medicines and the renewed official focus on strength training.

University College London research estimated that 1.6 million adults in England, Wales and Scotland used GLP-1 medicines to help lose weight between early 2024 and early 2025.

That is a historic usage estimate, not proof that all 1.6 million remain on treatment today. It nevertheless shows why exercise support around these medicines has become a significant public-health issue.

What Are Mounjaro, Wegovy and Other Weight-Loss Jabs?

Weight-loss jabs are prescription medicines that can reduce appetite, increase feelings of fullness and support weight management in eligible patients.

The phrase “weight-loss jab” is commonly used for several different medicines. They do not all contain the same ingredient, act in exactly the same way or have the same UK licence.

Brand Ingredient UK position
Mounjaro Tirzepatide Licensed for type 2 diabetes and weight management in eligible patients.
Wegovy Semaglutide Licensed for weight management in eligible patients alongside diet and physical activity.
Ozempic Semaglutide Licensed for type 2 diabetes rather than as a UK weight-management treatment.
Saxenda Liraglutide A liraglutide medicine licensed for weight management in eligible patients.

GLP-1 medicines mimic a naturally occurring hormone involved in appetite and blood-sugar control. Mounjaro acts on both GLP-1 and a second receptor known as GIP.

These are prescription medicines, not general cosmetic products. Decisions about starting, stopping, changing or switching treatment belong with an appropriately qualified healthcare professional.

This page covers the searches people commonly use — including Mounjaro muscle loss, Wegovy strength training, Ozempic exercise, tirzepatide resistance training and semaglutide lean mass — while keeping the main topic focused on the official exercise advice.

Do Weight-Loss Jabs Cause Muscle Loss?

Weight loss with Mounjaro, Wegovy and other GLP-1-based treatments can include a reduction in measured lean mass as well as body fat.

That does not mean the medicines automatically destroy muscle. Substantial weight loss rarely comes entirely from stored body fat. Some non-fat tissue may also decrease during dieting, medication-supported weight loss or after bariatric surgery.

The evidence is more precise than many dramatic headlines suggest:

  • Most weight lost in the major trials was body fat
  • Measured lean mass also decreased in some studies
  • Lean mass is not the same as skeletal muscle
  • Body-composition scans do not directly measure strength
  • Results vary between medicines, studies and individuals
  • Age, nutrition, starting fitness, activity and rate of weight loss can affect the outcome

The sensible response is not to panic or stop prescribed treatment without medical advice. It is to ensure that weight loss is supported by appropriate resistance exercise, nutrition and regular movement.

Lean Mass Is Not the Same as Skeletal Muscle

One of the most important distinctions in this discussion is the difference between lean mass and muscle.

Term What It Means Why It Matters
Fat mass Body tissue classified as fat. Reducing excessive body fat is normally the intended goal of medical weight management.
Lean mass Non-fat tissue including muscle, water, organs and connective tissue. A reduction in lean mass cannot automatically be described as the same quantity of muscle loss.
Skeletal muscle Muscles used to create movement and force. Supports strength, posture, balance, mobility and physical independence.
Muscle strength The ability to produce force. Strength can change differently from muscle size or total lean mass.
Physical function The ability to complete movements and daily tasks. Walking, lifting, balance and independence may matter more than one scan result.

This is why claims such as “25% of the weight lost is muscle” or “40% of the weight lost is muscle” should be treated cautiously. Those figures may refer to measured lean tissue, not pure skeletal muscle.

What Do the Semaglutide and Tirzepatide Studies Show?

Semaglutide and the STEP 1 Body-Composition Analysis

The STEP 1 body-composition analysis examined changes in adults with overweight or obesity receiving semaglutide.

Participants experienced substantial reductions in body weight, total fat mass and visceral fat. Total measured lean mass also fell.

However, lean mass represented a greater proportion of participants’ lower final body weight because body fat decreased more substantially.

The result therefore needs context: absolute lean mass decreased, but overall body composition shifted towards a lower percentage of body fat.

Tirzepatide and the SURMOUNT-1 DXA Substudy

In the SURMOUNT-1 body-composition substudy, approximately:

  • 75% of the weight lost was fat mass
  • 25% of the weight lost was lean mass

That does not mean 25% was pure muscle. Lean mass also includes body water, organs and other non-fat tissue.

The researchers found broadly similar fat-to-lean-loss proportions in the placebo group, supporting the view that some lean-tissue reduction is a recognised feature of substantial weight loss rather than automatically a unique damaging effect of tirzepatide.

What the Research Does Not Yet Prove

  • That everyone taking a GLP-1 medicine will lose the same proportion of lean mass
  • That all measured lean-mass loss is skeletal muscle
  • That one exercise programme will prevent all lean-tissue loss
  • That boxing alone specifically prevents GLP-1-related muscle loss
  • That a body-composition result tells us everything about strength or physical function

The practical lesson is to monitor strength, fitness and everyday function alongside body weight.

Should You Strength Train While Taking Mounjaro?

Yes. People taking Mounjaro for weight management should include appropriate muscle-strengthening exercise as part of their wider activity programme.

Mounjaro is the brand name for tirzepatide, a medicine acting on both GIP and GLP-1 receptors.

In the UCL research estimating weight-loss-drug use across Britain, four out of five respondents using medication exclusively for weight loss reported using Mounjaro. This makes practical exercise advice around Mounjaro particularly relevant.

A balanced Mounjaro exercise programme may include:

  • Strengthening all major muscle groups on at least two days each week
  • Squats or sit-to-stands for the legs
  • Hip hinges, bridges or suitable deadlift variations
  • Press-ups or resistance presses
  • Rows and other pulling exercises
  • Loaded carries and controlled trunk work
  • Walking or another manageable aerobic activity
  • Boxing technique and padwork at an appropriate intensity

The precise programme should reflect your health, current fitness, symptoms, previous injuries and advice from your clinical team.

Ask About Mounjaro-Friendly Exercise

Should You Strength Train While Taking Wegovy?

Yes. Strength training is important while taking Wegovy because substantial semaglutide-supported weight loss can include reductions in lean tissue as well as body fat.

Wegovy is the weight-management brand of semaglutide. Ozempic contains the same active ingredient but has different UK licensing and is authorised for type 2 diabetes rather than weight management.

Resistance training cannot guarantee that no lean tissue will be lost. It can, however, provide an important stimulus to maintain strength and physical capacity during weight reduction.

Boxing can complement the strength work by providing:

  • Cardiovascular exercise
  • Balance and controlled footwork
  • Coordination and reaction
  • Whole-body movement
  • Skill development
  • A structured and enjoyable reason to remain active

The combination is more complete than relying only on walking, conventional cardio or repeated pad rounds.

Why Does Muscle Strength Matter During Weight Loss?

Muscle strength is not only relevant to athletes or bodybuilders. It supports:

  • Walking and climbing stairs
  • Getting up from a chair or the floor
  • Carrying shopping and luggage
  • Posture and joint stability
  • Balance and reduced fall risk
  • Maintaining independence as you age
  • Training safely and confidently

A successful weight-management programme should ideally make someone more capable, not simply lighter.

Useful progress questions include:

  • Are you maintaining or improving your strength?
  • Can you walk further without excessive fatigue?
  • Has your balance improved?
  • Are everyday tasks becoming easier?
  • Are you recovering more comfortably after exercise?
  • Have you developed an activity routine you enjoy enough to continue?

How Can Boxing Help People Taking Weight-Loss Jabs?

Boxing can make physical activity more engaging by combining cardiovascular exercise with balance, coordination, footwork, skill and whole-body movement.

A properly structured session may include:

  • Stance and posture
  • Footwork and controlled changes of direction
  • Padwork and punch combinations
  • Defensive movement
  • Rotation through the hips and trunk
  • Short cardiovascular intervals
  • Balance and coordination drills
  • Progressive resistance exercises

For many people, boxing is easier to remain motivated for than repetitive machine-based cardio. Learning a skill gives the session a purpose beyond simply burning calories.

However, boxing should not be oversold. There is not currently strong clinical evidence proving that boxing specifically prevents lean-mass loss during Mounjaro or Wegovy treatment.

Boxing is best used as the cardiovascular, movement and motivation component of a programme that also includes genuine strength training.

“Weight-loss medication may help change the number on the scales. Boxing and resistance training can help someone become fitter, stronger and more confident in what their body can do.”

— Paul Carroll, Boxing Trainer London

Boxing Is Not Automatically a Complete Strength Programme

Fast punching can build muscular endurance in the shoulders and arms. Footwork challenges the legs, and rounds improve cardiovascular fitness.

But padwork alone may not progressively load the legs, hips, back, chest and pulling muscles enough. A complete programme should include separate resistance exercises.

What Strength Exercises Work Best With Boxing?

A balanced programme should include the major movement patterns rather than random circuits selected only to make the client tired.

1) Squatting Exercises

Examples include:

  • Supported sit-to-stands
  • Bodyweight squats
  • Goblet squats
  • Controlled split squats

These strengthen the legs and support activities such as climbing stairs, getting up from a chair and maintaining a stable boxing stance.

2) Hip-Hinge Exercises

Examples include:

  • Glute bridges
  • Kettlebell deadlifts
  • Resistance-band hinges
  • Controlled Romanian deadlifts

These train the hips and posterior chain, supporting lifting ability, posture and force transfer through the body.

3) Pushing Exercises

Examples include:

  • Wall press-ups
  • Incline press-ups
  • Floor press-ups
  • Resistance-band chest presses
  • Dumbbell presses

Pushing exercises strengthen the chest, shoulders and arms while allowing the resistance and range of movement to be adapted to the individual.

4) Pulling Exercises

Examples include:

  • Resistance-band rows
  • TRX or suspension-trainer rows
  • Supported dumbbell rows

Pulling exercises are particularly useful alongside boxing because they help balance repeated forward punching movements and support upper-back strength, shoulder stability and posture.

5) TRX and Suspension Training

Client performing a TRX chest press during boxing and strength training with Paul Carroll in London
TRX chest presses provide adjustable resistance for the chest, shoulders, arms and core during a combined boxing and strength-training programme.

TRX training works particularly well alongside boxing because the resistance can be adjusted simply by changing body position. This makes it suitable for complete beginners as well as stronger, more experienced clients.

Useful TRX exercises include:

  • TRX squats: strengthen the legs while allowing additional support and control
  • TRX split squats: develop single-leg strength, stability and balance
  • TRX rows: strengthen the upper back and help balance repeated punching movements
  • TRX chest presses: provide adjustable pushing resistance for the chest, shoulders and arms
  • TRX hamstring curls: strengthen the hamstrings, hips and posterior chain
  • TRX fallouts: challenge trunk stability and controlled core strength
  • TRX assisted lunges: develop leg strength while providing support for balance

TRX exercises require the body to remain controlled while the straps move. This challenges balance, posture and trunk stability, all of which can support better boxing stance, footwork and force transfer.

The aim is not to perform unstable exercises simply because they look difficult. The straps should be used to improve movement quality, adjust resistance and help the client progress safely.

6) Loaded Carries

Farmer’s carries and suitcase carries train grip, posture, trunk stability and practical whole-body strength.

They can also help a client learn to maintain good posture while moving under load, which has useful carry-over to boxing stance and general physical function.

7) Trunk and Core Stability

Examples include:

  • Dead bugs
  • Bird dogs
  • Pallof presses
  • TRX fallouts
  • Loaded carries
  • Controlled bracing exercises

The aim of core training is not simply to complete large numbers of sit-ups. It is to improve the ability to control the trunk, transfer force between the lower and upper body and remain stable while punching and moving.

The aim is not to complete every exercise in one session. The aim is to select appropriate movements, learn them correctly and increase the challenge gradually.

What Could a Boxing and Strength Session Look Like?

Session Section Example Time Main Purpose
Warm-up and readiness check 8–10 minutes Raise body temperature and assess energy, mobility and symptoms.
Boxing technique 10–15 minutes Practise stance, footwork, guard and controlled combinations.
Strength training 20–25 minutes Train the legs, hips, pushing, pulling and trunk muscles.
Padwork or boxing intervals 10–15 minutes Improve boxing skill, cardiovascular fitness and confidence.
Cool-down 5 minutes Reduce intensity gradually and review the session response.

A beginner does not need to finish every session exhausted. The early priorities should be sound movement, safe progression and consistency.

Example Weekly Boxing and Strength Plan

Boxing and strength training session with Paul Carroll and a female client in London
Private boxing training can combine cardiovascular fitness, movement, coordination and progressive resistance exercise.

Beginner Weekly Structure

  • Monday: boxing technique and full-body strength training
  • Tuesday: comfortable walk and regular movement breaks
  • Wednesday: rest, mobility or gentle activity
  • Thursday: padwork and full-body strength training
  • Friday: walking, cycling or another manageable aerobic activity
  • Weekend: one longer walk or recreational activity, depending on recovery

This is an example, not a medical prescription. Frequency and intensity should reflect the individual’s health, symptoms, experience and clinical advice.

Ask Paul About a Personal Training Plan

How Hard Should You Train?

People returning to exercise should normally begin below their maximum capacity and progress gradually.

Trying to compensate for years of inactivity with punishing workouts is unnecessary and may make consistency less likely.

Progress can come from:

  • Improving exercise technique
  • Completing one additional controlled repetition
  • Using slightly more resistance
  • Increasing the range of movement where suitable
  • Completing an additional boxing round
  • Improving footwork and balance
  • Recovering more comfortably between rounds
  • Moving more frequently outside formal sessions

The aim is progressive training, not punishment.

Protein, Nutrition and Hydration

Resistance training works best when the body also receives appropriate nutrition and sufficient fluid.

Because GLP-1 medicines can reduce appetite, some people may struggle to consume enough food or maintain a balanced diet. The quality of the food eaten therefore matters.

Protein contributes to maintaining muscle, but there is no single online protein target suitable for everyone. Requirements can differ according to:

  • Age
  • Body size
  • Activity level
  • Target weight
  • Total food intake
  • Medical history
  • Kidney health

A registered dietitian or the clinical team responsible for treatment can provide individual nutrition advice. An exercise coach should not diagnose nutritional deficiencies or prescribe a medical diet.

Persistent vomiting, diarrhoea or difficulty keeping fluids down can cause dehydration. A demanding boxing workout is not appropriate when someone is significantly dehydrated or acutely unwell.

Training Around Mounjaro, Wegovy and GLP-1 Side Effects

Training may need to be adjusted when symptoms such as nausea, vomiting, diarrhoea, fatigue, dizziness or reduced food intake affect how someone feels.

Tell your trainer when:

  • You have recently started treatment
  • Your dose has recently changed
  • You are experiencing nausea, vomiting or diarrhoea
  • You have eaten or drunk substantially less than usual
  • You feel unusually weak, dizzy or unwell
  • Your prescriber has given you specific exercise advice

The session can then be adapted by reducing intensity, increasing rest, changing the exercise selection or postponing demanding work.

When to Seek Urgent Medical Advice

Seek urgent medical attention for severe, persistent stomach or abdominal pain, particularly pain spreading towards the back and accompanied by nausea or vomiting. The MHRA identifies this as a possible symptom of acute pancreatitis.

Anyone taking semaglutide who experiences sudden blindness or a rapid deterioration in eyesight should also seek urgent medical help.

Do not stop or alter prescribed medication without discussing it with the responsible healthcare professional.

Can Exercise Help After Weight-Loss Medication Is Stopped?

Exercise cannot guarantee that weight will not return, but it can help establish the strength, fitness and routines needed for longer-term weight management.

The updated Chief Medical Officers’ guidance says strength activity is likely to be particularly important for people currently taking or recently finishing GLP-1 medicines.

A randomised trial involving liraglutide found that combining structured exercise with medication supported healthier weight-loss maintenance than either intervention alone.

The trial involved liraglutide, not every current medicine, so it should not be treated as proof that all Mounjaro or Wegovy users will achieve the same result.

It does support a wider principle: medication and exercise should not be viewed as competing choices.

Medication may support weight reduction. Exercise develops strength, movement, fitness, confidence and habits that can continue beyond treatment.

What Should You Measure Besides Body Weight?

Useful measures include:

  • Strength used for squats, rows and presses
  • Controlled sit-to-stand performance
  • Walking distance or daily steps
  • Waist measurement
  • Balance and movement confidence
  • Recovery after a boxing round
  • Energy during everyday activities
  • How clothing fits
  • Consistency of weekly exercise

Consumer body-composition scales can fluctuate considerably according to hydration and other variables. They should not be treated as precise clinical measurements of muscle mass.

Why 1-to-1 Boxing and Strength Coaching Can Help

A large class may require everyone to complete the same session. Private coaching allows the exercises, rounds, resistance and rest periods to be adjusted to the individual.

In a 1-to-1 session, I can adapt:

  • The complexity of boxing combinations
  • The pace and duration of pad rounds
  • The amount of jumping or impact
  • The resistance used for strength exercises
  • The balance between boxing and resistance training
  • The duration of recovery periods
  • Movements around previous injuries or mobility limitations

No sparring is required. Sessions can focus entirely on boxing technique, padwork, movement, strength and fitness.

Weight Loss Jabs and Strength Training – FAQs

What is the new UK advice for people taking weight-loss jabs?

The updated UK Chief Medical Officers’ guidance says people taking GLP-1 medicines for weight management or diabetes especially need activities that maintain muscle strength. Adults should strengthen the major muscle groups on at least two days each week and remain regularly active.

Do weight-loss jabs cause muscle loss?

Weight loss with GLP-1-based medicines can include a reduction in lean mass as well as body fat. Lean mass is not identical to skeletal muscle, and the amount varies between medicines, studies and individuals.

Does Mounjaro cause muscle loss?

Weight loss with Mounjaro can include reductions in lean mass as well as fat mass. In a SURMOUNT-1 body-composition substudy, approximately 75% of weight lost was fat mass and 25% was lean mass. Lean mass is not the same as pure muscle.

What strength training should I do while taking Mounjaro?

A balanced programme should include squatting, hinging, pushing, pulling, carrying and controlled trunk exercises. Exercise selection and resistance should reflect your experience, health, symptoms and clinical advice.

Can I do boxing while taking Mounjaro?

Many people can participate in appropriately adapted boxing while taking prescribed Mounjaro. Begin gradually, include genuine resistance exercise and reduce the intensity if nausea, dehydration, dizziness or unusually low energy is present.

Does Wegovy cause muscle loss?

Semaglutide-supported weight loss can include a reduction in measured lean mass. In the STEP 1 analysis, body fat decreased more substantially than lean mass, meaning lean mass represented a greater proportion of the lower final body weight.

Can I do boxing while taking Wegovy?

Boxing can provide cardiovascular activity, balance, coordination and conditioning while taking Wegovy. Padwork should be combined with progressive strength exercises when maintaining strength is a key goal.

Is Ozempic the same as Wegovy?

Ozempic and Wegovy both contain semaglutide, but they are different branded products with different UK licences. Wegovy is licensed for weight management in eligible patients, while Ozempic is licensed for type 2 diabetes.

Does boxing count as strength training?

Boxing challenges several muscle groups, but padwork alone may not provide enough progressive resistance for all the major muscles. A complete plan should include exercises such as squats, hinges, presses, rows and loaded carries.

How often should someone taking a GLP-1 medicine strength train?

UK guidance recommends strengthening all the major muscle groups on at least two days each week. The exact programme should reflect the individual’s age, health, ability, symptoms and recovery.

Can a complete beginner start boxing while taking weight-loss medication?

Many beginners can start suitably adapted boxing exercise. Medication side effects, health conditions and a long period of inactivity may require additional caution. Begin with controlled technique, manageable rounds and basic resistance work.

Should I tell my personal trainer that I take Mounjaro or Wegovy?

Yes. Telling your trainer about relevant medication, symptoms and clinical advice can help them adapt the session. A personal trainer should not make decisions about your medicine or dose.

Can strength training prevent all muscle loss?

No exercise programme can guarantee that no lean tissue will be lost during substantial weight reduction. Progressive resistance exercise can support muscle strength and function, especially when combined with appropriate nutrition and clinical care.

How many people use weight-loss jabs in Britain?

UCL research estimated that 1.6 million adults in England, Wales and Scotland used GLP-1 medicines to help lose weight between early 2024 and early 2025. This is a historic usage estimate rather than a current live count.
